Terms and Conditions
Our obligations:
- We are a private diagnostic service for autism and ADHD offering pre and post diagnostic support
- We provide a service from 13 years old
- We provide “gold standard” assessments and will provide you with a detailed report of our findings. An assessment does not mean you will automatically be diagnosed
- We are not obliged to accept you as a client if we feel we are not the right service. We do not need to give a reason as to why we haven’t accepted you and can cease work if we judge it is no longer appropriate.
Your obligations, you agree that:
- You will supply full and accurate information
- You will notify us of any changes to your details, especially if these may affect suitability or safety for the service
- You will notify us immediately if you need to cancel or reschedule any appointments. We require at least 48 hours notice for any cancellations.
- A client agreement form will be completed prior to assessment. Assessments cannot be booked until this is received and completed in full
- You provide us with any relevant documentation which may be needed to support the assessment such as medical records so we have all information we need to complete the assessment.
- A non-refundable deposit of £200 must be paid to secure your assessment date.
- You must pay the outstanding balance 2 weeks before your assessment.
Cancellation and lateness:
- We understand it is sometimes necessary to cancel appointments. Please give 48-hour notice if you need to cancel for any reason.
- Portland assessments has a 48-hour cancellation policy, booking cancellations made less than 48 hours before will not be eligible for refunds.
- If we have to cancel the appointment for any reason we endeavour to reschedule as soon as possible with no extra cost
- If you are more than 30 minutes late to your appointment without informing us, we may consider this a nonattendance in which case you won’t be eligible for a refund
Accessibility
- We strive to have a neurodiverse friendly environment with experienced staff to support with this, however we are based at a specialist college and therefore there may be learners and citizens in the environment who can sometime be noisy/unpredictable. We also have the public visiting our facilities so please be aware of this. You will always have someone with you when on campus who is experienced with our young people.
- If you or those attending the assessment have any accessibility needs, please inform us before your appointment so we can ensure these needs are supported
- We have multiple locations where the assessments can take place and we can discuss home assessments, please contact us to discuss options.
- You will need to sign in at reception in line with our safeguarding policies and wear a visitor badge.
- If you need additional support such as signing, translator services or personal care, please arrange this prior to your assessment.
Confidentiality and consent
- We will manage your personal data in accordance with all relevant legislation including UK GDPR
- All information will be held in the strictest confidence and will not be shared with any third parties without your written consent.
- There are some exceptional circumstances where we may be legally required to share information for example, the information you disclose shows a serious risk to yourself or others, information regarding terrorism or serious crimes or where we are legally required in accordance with child protection legislation to report safeguarding/child protection concerns.
Children and young people
- As we offer assessments for individuals under 18 years old, we will require consent from an individual with parental responsibility to undertake the assessment.
- As a person-centred assessment provider, we will seek consent from all of our young people seeking assessment, the young people will have opportunity to discuss their expectations from the assessment and can make decisions regarding their participation in the assessment including refusal to engage.
- All our staff are DBS checked and adhere to our Safeguarding Policy
Complaints
- We operate a zero tolerance policy to any forms of abuse and all parties have the right to stop the assessment at any point if they feel this policy is being breached.
- We value our client’s feedback and we would like to hear if the service was not to your satisfaction. A feedback form will be provided at the end of your assessment.
